Primary Care
Who it is for
Patients of all ages who are rostered to a Generations Medical Centre provider, from newborns to older adults. We focus on attaching people in our community who do not currently have a Family Doctor or Nurse Practitioner.
How to access this service
Primary Care is for rostered patients. To join, see Become a Patient to check eligibility and register. Once you are a patient, you can book through our online booking tool or by phone.

What it costs
Primary Care visits are covered by OHIP. Bring your valid Ontario health card to every appointment. Some items that OHIP does not cover, such as certain forms, notes, or vaccines, may have a fee, and we will tell you in advance.
What is included
- Routine and preventive care, including check-ups, immunizations, and cancer and chronic-disease screening.
- Diagnosis and treatment of everyday illnesses and injuries.
- Ongoing management of chronic conditions such as diabetes, high blood pressure, and heart disease.
- Prescriptions and prescription renewals.
- Referrals to specialists and to our allied health and mental health teams.
- Connection to social and community supports where they affect your health.
Your care team
Care is provided by family physicians, Nurse Practitioners, and physician assistants, working alongside nurses and the wider team. Because everyone shares one electronic medical record, your information moves with you across the team and you do not have to repeat your story.
