Who it is for
Patients living with or at risk of chronic conditions such as diabetes, cardiovascular disease, or related concerns.
How it works
The program is built on evidence-based protocols and often runs as a series of sessions. Your provider can refer you, or you can ask our team whether the program is right for you.

What it costs
Program participation is generally covered for rostered patients. We will let you know in advance if any element is not covered.
What is included
- Group and individual education on managing your condition day to day.
- Support setting and tracking personal health goals.
- Foot care and other targeted services for higher-risk patients.
- Coordination with your Primary Care provider and, where helpful, allied health and dietitian support.
Your care team
Care is provided by family physicians, Nurse Practitioners, and physician assistants, working alongside nurses and the wider team. Because everyone shares one electronic medical record, your information moves with you across the team and you do not have to repeat your story.
