What it costs
Medical care is covered by OHIP. Some community or home-based supports may be delivered by partners with their own coverage, which we will help you understand.
What is included
- Comprehensive care for the complex and overlapping needs that can come with age.
- Foot care and preventive services.
- Coordination with the long-term care, housing, and community supports within Generations Toronto.
- Connection to home and community services where appropriate.
Your care team
Care is provided by family physicians, Nurse Practitioners, and physician assistants, working alongside nurses and the wider team. Because everyone shares one electronic medical record, your information moves with you across the team and you do not have to repeat your story.
