What it costs
Most community health activities are free. Any exceptions will be noted when you register.
What is included
- Health promotion and prevention activities, including screening and education.
- Nutrition and healthy-living support.
- Connection to the wider Generations Toronto ecosystem, including food, early childhood, and intergenerational programming.
- Warm referrals to community organizations, settlement agencies, and social services.
Your care team
Care is provided by family physicians, Nurse Practitioners, and physician assistants, working alongside nurses and the wider team. Because everyone shares one electronic medical record, your information moves with you across the team and you do not have to repeat your story.
